Case Management is a program that provides support and individualised service delivery to people in the Hume Region. Services are arranged in a manner that provides holistic, person-centred care that enhances people's rights, dignity and independence. Case Management may include the following:
Individually planned and co-ordinated packages of care tailored to help older Australians remain living in their own homes. Services include: personal care, social support, transport to appointments, home help, meal preparation and gardening.
To receive a CACP, you must be assessed by an Aged Care Assessment Team (ACAT) as requiring low level care*.
Designed for people with high care needs. These packages can assist with: activities of daily living, continence management, mobility, home maintenance and other supports.
To receive an EACH Package, you must be assessed by an Aged Care Assessment Team (ACAT) as requiring high level care*.
UnitingCare Goulburn North East is a provider of Individual Support Packages which are designed to enable people with a disability to meet their goals and exercise choice in obtaining support that assists them in pursuing their own lifestyles. The funds may be used to buy a range of supports and services in response to individual needs, circumstances and choices which will also take into consideration the needs of family members or carers to maintain the caring relationship.**
