With the happy news from the Department of Health that the consortia of Westmont Aged Care Services Ltd. and UnitingCare Goulburn North East has been successful in their tender to deliver Home and Community Care (HACC) services in Wodonga, we are keen to assure all current HACC service users of our commitment to achieve a smooth transition. Activity Groups at "The Willows" will continue with the same or similar timetable; as will HACC services to be delivered by Westmont. Individual Support Planning Facilitation

This service gives people with disabilities the opportunity to have individually tailored and flexible support plans developed or reviewed.

The person with a disability is encouraged to write their own plan with the support of friends, family and our qualified, experienced local facilitator. People are encouraged to remain in and engage with the local community through established support networks.

Support Coordination

Support coordination is for people requiring low-level assistance to keep their supports running smoothly.

Enabling Independent Living - Housing

Enabling Independent Living aims to assist people with a range of disabilities, who are aged between 18 and 64 to explore accommodation options for independent living (either alone or with housemates).

UnitingCare Goulburn North East offers up to three months of support after moving.

Older Carers Respite Program

This service is for parents aged 60 years and over, living in North East Victoria or the Goulburn Valley and caring for an adult with a disability.

The program arranges in-home activities, trips and outings, camps, special interest activities, day and overnight respite and planned or emergency respite.

Support to Older Carers Program

The Support to Older Carers Program offers carers over 60 years of age (or 45 if indigenous) planning and support for their current situation and into the future.  The UnitingCare Goulburn North East Planner will discuss with the carers their current needs in order to connect them with in-home assistance and respite options. Future planning could include connecting with disability, aged care or social services, facilitating a Power of Attorney, a Will or making a financial or estate plan.

Outreach Support

Up to 15 hours per week of direct assistance is provided so that a person with a disability can live independently or develop skills that will enable the person to live independently in the future.

Supported Living Fund

The aim of the New South Wales Supported Living Fund (SLF) is to expand the range of accommodation support options to offer more choice to people with a disability, their families, carers and support networks.

The person is encouraged to write their own proposal, remain actively engaged in planning their program, and in purchasing supports which complement the persons existing informal / formal supports and networks.

UnitingCare Goulburn North East (UCGNE) is a registered SLF Financial Administrator. The role of the Financial Administrator is to manage the person's SLF funding for an agreed fee. Some tasks may include:

  • managing funding, purchasing supports and services;
  • providing information and support to the person with a disability and their family and carers;
  • providing a human resource function (legal employer of staff, insurances, work health and safety, workplace/Awards obligations); and
  • ongoing support to assist the person with a disability to implement and review their Support Plan.
